I have completed it on the first attempt with 1 hp left after losing 2 at the very beginning to the underground hands. As ZenBreaker said, something that will haunt you would be great for the game. It shouldn't necessary be well-designed zombie with pathfinding, a scary slow ghost that flying through the graves directly to the player will do the trick. Also it may appear after like 3 souls and become faster after every 2 next.
